Incorporating Social Determinants of Health into EHRs: Experience from the Field

Join national thought leaders from Kaiser Permanente and OCHIN at the Oregon Chapter of HIMSS Population Health Management Forum as they discuss their research on, and their organizations’ journeys incorporating social determinants of health (SDOH) into Electronic Health Records (EHRs).

With the awareness that SDOH - the non-clinical circumstances of patients' lives – can impact health more profoundly than clinical care there is a growing national emphasis on systematic documentation of SDOH in EHRs. However, there is little scientific evidence nor are there national guidelines on how healthcare providers can or should integrate SDOH into their EHRs and workflows.

The forum is open to HIMSS members, guests, and all those interested in learning more about SDOH and strategies for incorporating them into EHRs and workflows. Individuals who will find this forum beneficial include: informaticists, clinicians, pharmacists, healthcare analysts, administrators, Population Health Management leadership, and Social Determinants strategists. Bring your questions for this educational networking event, all attendees can share insights and ask questions in this interactive presenters/audience discussion.

Speakers

Rachel Gold, PhD, MPH

Senior Investigator and Lead Research Scientist
Kaiser Permanente Northwest Center for Health Research, and OCHIN, Inc.

Dr. Gold is a Senior Investigator at the Kaiser Permanente NW Center for Health Research, and the Lead Research Scientist at OCHIN, Inc. Dr. Gold's research evaluates methods for using health information technology to address health disparities. She is Principal Investigator of two of the first NIH-funded studies assessing the adoption of EHR-based documentation of patients' social determinants of health (SDOH) in community health centers, and of actions that these clinics take to help address identified social determinants. She serves on SIREN's (the Social Intervention Research and Evaluation Network) Research Advisory Committee, Kaiser Permanente's SONNET steering committee on SDH research, Epic's social determinants brain trust and Epic's Population Health Steering Committee, and the National Academies of Medicine committee on integration of social needs into medical care.


Briar Ertz-Berger, MD, MPH

Briar Ertz-Berger is an emergency medicine physician and the Assistant Director of Operations for Special Populations at KPNW. Dr. Ertz-Berger’s work focuses on eliminating health inequities and the effects of unmitigated socio-economic struggles. Her passion for this work started with serving patients in the Bronx during medical school and continued at OHSU during her residency and subsequent fellowship where she studied the impact of the Oregon Health Plan cuts on emergency room utilization for chronic conditions. Dr. Ertz-Berger works closely with partners within Care Delivery Strategy at KPNW, leading efforts to address the social, economic, and behavioral needs in order to improve members’ lives and to improve population health.
